What to Expect Upon Arrival to Your Appointment

Whether you are a new patient, current patient, or here for a procedure, you will be greeted by one of our staff at our clinic entrance. You will be required to complete the following:

  1. No-touch temperature read.

  2. Wear a mask upon entry.

  3. Complete screening questionnaire, signature required.

If you choose to opt out of any of the above, your appointment will be canceled - per federal EEOC regulations, workers have the right to feel safe in their place of work. The ADA allows access restrictions when an individual would pose a direct threat to the health or safety of others. In March 2020, the EEOC declared that the COVID-19 pandemic meets the “direct threat” standard. The accommodations we can provide at this time are telehealth (if appropriate) or referral.
